What Is an Organic Acids Test?
Organic acids are small molecules your body produces as it burns fuel, builds and breaks down neurotransmitters, and processes what your gut microbes make. An OAT collects a urine sample — usually first thing in the morning — and measures a panel of these compounds. Functional-medicine practitioners typically read the results in three broad territories:
- Microbial and yeast markers — compounds such as arabinose and tartaric acid, used as indirect proxies for yeast or bacterial overgrowth in the gut;
- Mitochondrial and energy markers — intermediates of the cell's energy-production cycle, read as clues about how efficiently cells are making energy;
- Neurotransmitter metabolites — breakdown products such as homovanillic acid (HVA, from dopamine), read as rough indicators of neurotransmitter turnover.
That range is the OAT's appeal: one urine sample gestures at metabolism, energy and brain chemistry at once. It is also the OAT's limitation — everything on the report is an inference from a metabolite, and interpretation conventions vary from lab to lab.

Indirect vs Direct: The Real Difference Between an OAT and a Stool Test
The most common reason people look up the OAT is a suspicion about gut yeast or bacterial overgrowth. Here it's worth being precise about what each test actually does.
An OAT infers. When urinary arabinose or tartaric acid runs high, the interpretation is "something — possibly yeast — may be producing this upstream." That is a legitimate hypothesis-generating signal, but it is one step removed from the organism, and diet and individual metabolism can move the numbers.
A qPCR stool test measures. The GI-MAP™ detects and quantifies the DNA of the organisms themselves — Candida and other fungi, bacterial pathogens, commensal and opportunistic flora, parasites, viruses, and H. pylori with its virulence factors — alongside gut-function markers such as calprotectin, secretory IgA and pancreatic elastase-1. To say it fairly in the other direction: no stool test measures mitochondrial energy intermediates or neurotransmitter metabolites. If your questions are genuinely metabolic — fatigue, mood or energy patterns with unremarkable digestion — the OAT covers territory the GI-MAP™ does not, and a practitioner may reasonably want both lenses.
What Is Homovanillic Acid (HVA)?
Homovanillic acid is the principal end-product of dopamine metabolism, excreted in urine. It has a firmly established conventional use: physician-ordered HVA testing is part of the workup for suspected neuroblastoma and certain other catecholamine-related conditions. On an OAT, HVA is read more loosely, as a marker of dopamine turnover. That reading is interpretive, not diagnostic — a high or low HVA on a functional panel is a conversation starter with your practitioner, not a diagnosis of anything.
What Is Arabinose?
Arabinose is a simple sugar (a monosaccharide) found widely in plants — fruit, vegetables and grains all contain it. OAT laboratories treat elevated urinary arabinose as a proposed marker of intestinal Candida overgrowth, on the theory that yeast in the gut produces it in quantity. The honest status of that theory: plausible, used clinically for years, and still resting on mixed evidence — recent meals can raise the value, and a high result does not establish that yeast is the source. When Candida itself is the question, measuring Candida directly in stool is the less ambiguous route.
Tartaric Acid: In Food vs On a Lab Report
Tartaric acid is best known from the kitchen — it is the natural acid of grapes and therefore of wine, and cream of tartar is one of its salts. Because it is so common in food, it makes an awkward lab marker: OAT panels list elevated tartaric acid as another possible yeast signal, but grape juice, wine or baked goods made with cream of tartar shortly before collection can raise it for entirely mundane reasons. A good practitioner will always ask what you ate before reading anything into this number.
